The efficacy of any PRP treatment begins with its preparation—a meticulous process that demands precision and a profound understanding of cellular biology. During a recent educational session with fellow researchers, we delved into the finer points of this procedure, reinforcing the importance of technique.
Why Precision Matters: Separating Plasma from Cellular Components
The goal of PRP preparation is to concentrate platelets from a patient’s own blood. These platelets are tiny, powerful cell fragments packed with growth factors—proteins that act as signaling molecules to orchestrate the body’s healing cascade.
- The Blood Draw and Centrifugation: The process starts with a simple blood draw, similar to a routine lab test. The blood is collected in special tubes containing an anticoagulant to prevent clotting. These tubes are then placed in a centrifuge, a machine that spins them at high speeds.
- The Science of Separation: The centrifugal force separates the blood into distinct layers based on density:
- Red Blood Cells (Erythrocytes): The heaviest components, which settle at the bottom.
- The “Buffy Coat”: A thin middle layer containing white blood cells (leukocytes) and, most importantly, the platelets.
- Platelet-Poor Plasma (PPP): The lightest component, a yellowish fluid that rises to the top.
A crucial element in modern PRP kits is the gel separator, a thixotropic gel that forms a physical barrier between the red blood cells and the plasma after centrifugation. Its purpose is to prevent the red blood cells from remixing with the plasma when the PRP is extracted.

This is where technique becomes paramount. When extracting the platelet-rich layer, it is vital to avoid disturbing the gel separator.
- The Risk of Contamination: If the tip of the aspiration needle touches or punctures the gel, it can create a channel for red blood cells to leak back into the plasma. As one researcher noted, “If the needle touches the gel, the other components can leak back in, and you will need to re-spin the sample.” Re-spinning is not ideal, as it can damage the delicate platelets and reduce the overall quality of the PRP.
- My Preferred Technique: To maintain purity and control, I advocate for a technique where the needle is kept just above the buffy coat layer, drawing the plasma from the source of the tip. By angling the needle and working slowly, I can harvest the maximum concentration of platelets without risking contact with the gel separator. As I’ve observed, “I have more control over it when it’s at the source of the tip.” It’s a safety measure to ensure the final product is pure, potent, and primed for healing.
This careful, science-backed approach ensures that the PRP we use is of the highest quality, containing a rich concentration of the growth factors necessary to stimulate tissue repair, reduce inflammation, and promote cellular regeneration.

- The Healing Mechanism: When PRP is injected into a damaged joint or tissue, the concentrated growth factors—such as Platelet-Derived Growth Factor (PDGF), Transforming Growth Factor-Beta (TGF-β), and Vascular Endothelial Growth Factor (VEGF)—are released. You might wonder how chiropractic care fits into this advanced, cell-based approach. The connection is fundamental.
