This pain stems from the irritation or compression of the greater and lesser occipital nerves. In this educational post, I will take you on a journey to understand this complex condition from an integrative perspective. We will explore the anatomy and physiology of the occipital nerves, discuss the underlying mechanisms of pain, and present a modern, evidence-based approach to treatment.
A key focus will be the occipital nerve block, a targeted injection procedure designed to provide rapid, effective pain relief. I will walk you through this procedure, explaining each step and its rationale. We will also discuss how this intervention is one piece of a larger, more comprehensive puzzle.

Understanding Occipital Neuralgia: More Than Just a Headache
When patients come to me describing a severe, sharp pain that feels like an electric shock at the back of their head, one of the first conditions I consider is occipital neuralgia. This isn’t a typical tension headache or a migraine, although it can sometimes be confused with them. The pain of occipital neuralgia follows a very specific path, tracing the distribution of the greater and lesser occipital nerves.
These nerves emerge from the upper cervical spine, specifically from the C2 and C3 nerve roots. They then travel upward, piercing through several layers of dense muscle and fascia at the base of the skull, an area known as the suboccipital region. From there, they spread out to provide sensation to the back and top of the scalp, extending almost to the forehead.
Pain arises when these nerves become inflamed, compressed, or irritated anywhere along their path. The most common site of entrapment is where the nerves pass through the powerful semispinalis capitis and trapezius muscles. Chronic tension, poor posture (like “text neck”), or direct trauma from a whiplash injury can tighten these muscles, effectively squeezing the nerves. This compression disrupts normal nerve signaling, leading to the characteristic symptoms:
- Intense, sharp, shooting, or jabbing pain: Often described as an electric shock.
- Aching or throbbing pain: This can be a constant background discomfort between the sharp attacks.
- Pain originating at the base of the skull: Typically radiates up one side of the head.
- Scalp tenderness: The scalp can become so sensitive that even brushing hair or lying on a pillow is painful.
- Pain behind the eye: The nerve pathways can sometimes refer pain to the area behind the eye on the affected side.
The Diagnostic Journey: Pinpointing the Source of Pain
Accurately diagnosing occipital neuralgia is crucial, as its treatment differs significantly from other headache types. The first step is always a thorough history and physical examination. I listen carefully to the patient’s description of the pain—its quality, location, and triggers.
The most telling part of the physical exam involves palpation of the suboccipital region. I use my fingertips to carefully apply pressure along the base of the skull, searching for the path of the occipital nerves. In a patient with occipital neuralgia, pressing on the irritated nerve will often reproduce the sharp, radiating pain they experience. This is known as a positive Tinel’s sign of the occipital nerve.
In a recent case on September 1, 2026, a patient presented with classic symptoms. As I palpated the area, I identified two distinct points of extreme tenderness directly over the nerve pathway.
- “Is that hurting right there?” I asked, applying gentle pressure.
- “That’s it,” the patient confirmed.
I then located a second spot.
- “There?”
- “Yes. Yes,” they replied, confirming the second trigger point.
These specific, reproducible trigger points are the bullseyes we need for targeted treatment. To ensure precision, I use a simple but effective technique. With a ballpoint pen, I make a firm indentation on the skin at the exact spot of maximum tenderness. This small divot, combined with a discreet ink mark just below it, serves as a precise guide for the upcoming procedure, especially in the hairline where visual markers can be lost.
The Occipital Nerve Block: A Targeted Intervention for Rapid Relief
For patients with acute, severe occipital neuralgia pain, an occipital nerve block can provide significant relief. This procedure involves injecting a small amount of medication around the inflamed, irritated occipital nerve. The goal is twofold: diagnostic and therapeutic. If the pain is significantly relieved after the injection, it confirms the occipital nerve as the source. Therapeutically, it provides immediate and often lasting relief.
The Medication Cocktail
The injectate I use is a carefully prepared mixture of two key components:
- Lidocaine: This is a local anesthetic. Its primary job is to provide immediate pain relief. It works by blocking the sodium channels in the nerve fibers. Nerve impulses, which are essentially electrical signals, rely on the flow of sodium ions. By blocking these channels, lidocaine stops the pain signals from traveling from the occipital nerve to the brain. The patient experiences numbness and a dramatic reduction in pain within minutes.
- Cortisone (a corticosteroid): This is a powerful anti-inflammatory agent. While lidocaine provides the immediate “block,” cortisone addresses the underlying inflammation irritating the nerve. Corticosteroids work at a cellular level to suppress the production of inflammatory chemicals like prostaglandins and leukotrienes. This reduces swelling, heat, and pressure on the nerve, providing longer-term relief that can last for weeks or even months.
The Procedure: A Step-by-Step Walkthrough
Once the tender points are marked, the procedure itself is quick and precise.
- Preparation: Patient safety and comfort are paramount. I start by meticulously cleaning the marked areas with alcohol swabs. In the hairline, using a broad-spectrum antiseptic like Betadine can be messy and is often unnecessary for a clean, subcutaneous injection, so a thorough alcohol prep is sufficient.
- Instrumentation: For this procedure, I select a 25-gauge, 1-inch needle. This fine-gauge needle minimizes patient discomfort during insertion, while its length is ideal for reaching the subcutaneous tissue surrounding the nerve without going too deep. I draw the lidocaine and cortisone mixture into a syringe, planning to use half of the volume for each of the two identified trigger points. Normally, I might use a topical freeze spray to numb the skin, but in the hairline, the spray can run down into the patient’s eyes or ears, so we proceed without it after getting the patient’s consent. “You’re going to feel a little bit of a sting here. Okay, is that all right?” I asked. The patient consented.
- The Injection: With the patient ready, I locate the first marked spot. “One, two, three,” I say, to help the patient anticipate the needle stick. I insert the needle until I feel it gently touch the occiput, the bony plate at the back of the skull. This bony backstop ensures I am at the correct depth and not too close to deeper, more sensitive structures.
- Aspiration (The Safety Check): Before injecting any medication, I perform a critical safety check called aspiration. I gently pull back on the syringe’s plunger. If blood enters the syringe, it means the needle tip is inside a blood vessel. Injecting the medication, particularly the corticosteroid, into a vessel could cause systemic side effects. Seeing no blood on aspiration (“nothing on aspiration”) confirms that the needle is in the correct tissue plane.
- Administering the Medication: With safety confirmed, I slowly and gently inject half of the syringe’s contents, bathing the irritated nerve in the anesthetic and anti-inflammatory solution. I then repeat the exact same process for the second trigger point, using the remaining medication.
Immediate Feedback and Post-Procedure Care
The effects of the lidocaine are almost instantaneous. Immediately after the injections, I gently massage the area to help the medication spread throughout the surrounding tissue and fully encompass the nerve. Then comes the moment of truth.
I apply firm pressure to the same spots that were excruciatingly painful just minutes before.
- “Now, does that hurt right there?” I asked, pressing on the first injection site.
- “Pressure,” the patient noted. “Is it painful?” I clarified. “Somewhat… It feels better.”
- I then moved to the second spot. “And then that spot right here. Is that painful?”
- “No, it feels better,” they confirmed.
- “Does that hurt at all to mash on it now?”
- “Just a little,” the patient replied.
- “So again, better than it was when you came into the room, right?”
- “Oh yes,” they said with relief.
This immediate, significant reduction in pain confirms the block’s success. The “just a little” pain remaining is typically from the needle stick itself and the pressure of the injected fluid, which will dissipate quickly.
Beyond the Injection: The Role of Integrative Chiropractic Care
The occipital nerve block is a powerful tool for breaking the cycle of acute pain, but it is not a cure on its own. It gives the patient a critical opportunity to engage in the rehabilitative care needed to address the root cause. This is where integrative chiropractic care becomes essential.
Occipital nerve compression is often a direct result of biomechanical dysfunction in the cervical spine and surrounding musculature. My approach as a chiropractor focuses on restoring proper function to this entire system.
- Chiropractic Adjustments: I use specific, gentle adjustments to the cervical vertebrae (neck bones). Misalignments in the upper cervical spine, particularly at the C1 (atlas) and C2 (axis) levels, can directly irritate the C2 nerve root from which the greater occipital nerve originates. Restoring proper alignment and motion to these joints can take direct pressure off the nerve root, providing fundamental relief.
- Soft Tissue Mobilization: The muscles that the occipital nerves pierce—the trapezius and semispinalis capitis—are often riddled with trigger points and adhesions. I use manual therapies like myofascial release, trigger point therapy, and instrument-assisted soft tissue mobilization (IASTM) to break down this scar tissue and release chronic muscle tension. This “un-entraps” the nerve as it passes through the muscle, allowing it to function without irritation.
- Postural Correction and Therapeutic Exercise: Poor posture is a primary driver of suboccipital muscle tension. We work with patients on ergonomic education and prescribe specific exercises to strengthen weak postural muscles (like the deep neck flexors) and stretch tight ones (like the upper trapezius and levator scapulae). This retrains the body to maintain a healthy head and neck position, preventing the recurrence of nerve compression.
