Find out how HRT and integrative medicine combine traditional and holistic approaches for optimal symptom relief from menopause.
In this educational post, I take you through a biology-first, integrative roadmap for perimenopause and menopause care that goes well beyond hormone replacement therapy (HRT). While HRT can relieve symptoms for many women, it does not correct core physiological problems like chronic inflammation, oxidative stress, NAD+ decline, insulin resistance in central and peripheral tissues, or mitochondrial dysfunction. I present a comprehensive program rooted in functional medicine, internal medicine oversight, and integrative chiropractic care to restore cellular energy, stabilize neurochemistry, improve thermoregulation, and address cardiometabolic risk. I explain why symptoms such as hot flashes, brain fog, weight gain, mood instability, and low libido often persist despite HRT alone and how targeted interventions—dietary strategies, micronutrient repletion, mitochondrial support, circadian alignment, autonomic nervous system regulation, and physical rehabilitation—work synergistically to resolve root causes. I also detail our multidisciplinary workflow at Injury Medical Clinic PA (Mission Plaza Injury Medical Clinic) in El Paso, Texas, where I collaborate with Dr. Maria Guadalupe Cardenas, MD (Board Certified in Internal Medicine; NPI #1164426749; Texas MD License #J2933), who serves as our Medical Director and Collaborative Physician, ensuring safe, evidence-based care that integrates medical oversight with chiropractic and functional medicine. This post includes practical protocols, clinical pearls, and research insights from leading investigators, with careful explanations of mechanisms, implementation steps, and monitoring plans.

I am Dr. Alex Jimenez, DC, APRN, FNP-BC, CFMP, IFMCP, ATN, CCST. My clinical approach bridges chiropractic care, advanced nursing practice, and functional medicine. In our practice—Injury Medical Clinic PA (also known as Mission Plaza Injury Medical Clinic) in El Paso, Texas—I work closely with Dr. Maria Guadalupe Cardenas, MD, Board Certified in Internal Medicine (NPI #1164426749; Texas MD License #J2933). Dr. Cardenas has over 40 years of experience as an internist and serves as our Medical Director and Collaborative Physician.
This multidisciplinary setup is common in integrative and injury care clinics: the MD provides medical direction, diagnostic oversight, and pharmacologic guidance; the chiropractor leads neuromusculoskeletal care, autonomic regulation strategies, and functional movement rehabilitation; and our functional medicine arm coordinates nutrition, lifestyle, and targeted supplementation. Together, we deliver comprehensive care that aligns medical safety with whole-person, biology-first strategies.

Many women are told their symptoms are “hormonal.” The reality is more nuanced. HRT can supply estradiol, progesterone, and sometimes DHEA, but if the biological terrain is inflamed, energy-deficient, and insulin-resistant, the body cannot translate hormones into healthy function. In practice, I have seen patients continue to experience hot flashes, fatigue, brain fog, mood instability, weight gain, and low libido despite “optimal” hormone labs. The reason: the core energy systems and signaling pathways are impaired.
Taken together, these factors explain persistent symptoms despite hormone therapy. The solution is a biology-first sequence that stabilizes cellular energy, reduces inflammatory noise, re-trains metabolic flexibility, and then uses hormones judiciously (with careful titration and monitoring).
Menopause brings a profound shift in redox and energy balance. Evidence indicates that NAD+—a central cofactor for mitochondrial respiration, sirtuin signaling, and DNA repair—declines significantly as ovarian hormone production wanes. When NAD+ drops, complex I function, beta-oxidation, and pyruvate dehydrogenase flux suffer. Cells produce less ATP, increasing reactive oxygen species (ROS), compromising mitochondrial membrane potential, and impairing calcium handling.
In brain regions like the prefrontal cortex and hypothalamus, this energy deficit translates to poor neurotransmitter synthesis (serotonin, dopamine, GABA) and dysregulated thermoregulation. The prefrontal cortex, which governs mood stability, executive function, and impulse control, becomes energy-starved. So even if antidepressants block serotonin reuptake, there may not be enough ATP and precursor flow to synthesize and package neurotransmitters adequately. The result: persistent depression, anxiety, irritability, and brain fog.
The hypothalamus integrates thermal signals and orchestrates vasomotor responses. Local insulin resistance—exacerbated by inflammatory cytokines and altered estrogen signaling—disrupts glucose sensing and energy utilization. Neurons become glucose-saturated but cannot oxidize it efficiently, producing a mismatch between energy demand and supply. The thermoregulatory set-point becomes erratic; vasodilatory signals misfire; hot flashes emerge as the system oscillates between heat dumping and shivering responses.
Menopausal transition impairs metabolic switching—the ability to toggle between glucose and fat oxidation. Insulin resistance in liver, muscle, and adipose tissue blocks fat mobilization while favoring visceral fat deposition. Mitochondrial beta-oxidation is restrained; carnitine shuttling is inefficient; and lipolysis signals are muted.
Our program begins with stabilizing the biological terrain. When the body’s energy systems function, we can titrate hormones to effective, steady states without provoking instability.
Under Dr. Cardenas’s medical direction, we obtain baseline labs and then follow-up assessments to guide care and ensure safety. We emphasize:
We repeat labs about 6 weeks after starting protocols to gauge response and adjust safely.
We implement dietary and supplement strategies to bring CRP and oxidative stress down quickly, as these interfere with vascular responses and neurotransmission.
Rationale: Lowering inflammatory tone restores endothelial responsiveness, improves hypothalamic signal fidelity, and reduces neurotransmitter turnover strain, allowing hormones and neuromodulators to function properly.
We focus on rebuilding mitochondrial capacity and restoring NAD+ pools to power cellular repair, thermoregulation, and neurochemical stability.
Rationale: ATP restoration underpins neurotransmitter synthesis, autonomic stability, and thermoregulation. NAD+ repletion improves sirtuin signaling, enhances stress resilience, and supports DNA repair, reducing the cellular “noise” that disrupts menopausal physiology.
We repair glucose-fat switching so cells can access and burn fuel effectively, reversing visceral fat accumulation driven by impaired oxidation.
Rationale: Restoring insulin sensitivity reduces glucose “soak” at the hypothalamus and metabolic tissues, enabling mitochondrial oxidation, stabilizing thermoregulation, and allowing adipocytes to release fat. This changes body composition without fixating on calories.
Hormones require time to achieve tissue steady state. Under Dr. Cardenas’s supervision, we apply a structured HRT protocol and avoid frequent dose changes that destabilize vasomotor control.
Rationale: Respecting pharmacokinetics prevents oscillations in vasomotor tone. Complementary nutrients provide the raw materials for proper steroid metabolism, neurochemistry, and vascular function.
Symptoms like depression, anxiety, and irritability often reflect energy-deficient brain networks rather than isolated “low serotonin.” Antidepressants can help some patients, but they may fail when neurons lack ATP to synthesize and package neurotransmitters. Our approach:
Rationale: When energy systems are restored, neurotransmitter synthesis and regulation improve, allowing serotonergic and dopaminergic pathways to reach functional stability.
Hot flashes arise from disturbed hypothalamic set-point control, endothelial dysfunction, and autonomic instability. Our multi-pronged strategy:
Rationale: Thermoregulation is an energy-dependent control loop. When ATP and vascular signaling normalize, hot flash frequency and intensity diminish.
Menopausal weight changes frequently reflect impaired fat oxidation and insulin resistance, not simply caloric excess. Our program addresses:
Rationale: Once metabolic flexibility returns, visceral fat decreases, lean mass improves, and weight stabilizes even without extreme caloric restriction.
Low libido, sleep disruption, and brain fog are energy and signaling problems:
