I will discuss the latest findings from leading researchers. Furthermore, I will explain the two primary devices used—Tenex and TenJet—which debride and remove damaged tendon tissue. Today, I am excited to share insights into a groundbreaking procedure. It has become an essential tool in our practice for managing chronic tendon pain: percutaneous tenotomy. This post will take you on a journey to understand what this procedure is, how it works, and why it represents a significant leap forward in treating conditions that were once notoriously difficult to resolve.

Unlocking a Solution for Chronic Tendon Pain

For years, patients suffering from chronic tendinopathies like tennis elbow (lateral epicondylitis), plantar fasciitis, or Achilles tendinopathy have navigated a frustrating cycle of pain, inflammation, and dysfunction. The underlying issue in these conditions is not just inflammation, but tendinosis—a degenerative state in which the tendon’s collagen fibers become disorganized, weakened, and filled with scar tissue. This damaged tissue is a primary source of chronic pain. It fails to heal on its own.

Traditionally, treatments ranged from rest and physical therapy to corticosteroid injections and, ultimately, open surgery. While these treatments can be effective for some, they often do not provide lasting relief. For example, corticosteroid injections can provide temporary relief but may weaken the tendon over time. This potentially leads to further damage (Coombes et al., 2010). Open surgery, while effective, involves significant downtime, risks of infection, and scarring.

Percutaneous tenotomy is where it comes in. It is a minimally invasive, FDA-approved procedure specifically designed to target and remove the source of the pain—the degenerated tendon tissue—without the need for open surgery. This technique is now accessible to trained primary care providers like me. It allows us to offer this advanced treatment directly in an office setting.

How Does Percutaneous Tenotomy Work? The Science Behind the Technique

Percutaneous tenotomy is performed under real-time ultrasound guidance, which is a cornerstone of its safety and precision. Ultrasound lets the practitioner visualize the anatomy in detail, pinpoint the exact location of the diseased tissue, and guide a specialized instrument to that area while avoiding healthy surrounding structures like nerves and blood vessels.

There are currently two leading technologies on the market for performing this procedure:

  • Tenex Health TX: This system uses ultrasonic emulsification, a concept borrowed from advanced cataract surgery. A small, 18-gauge double-lumen needle is inserted through a tiny incision (usually requiring only a sterile strip for closure).
    • The Science: The needle tip vibrates at an ultrasonic frequency (approximately 20,000 times per second). This focused energy precisely breaks down, or emulsifies, only the targeted fibrotic and calcified scar tissue.
    • The Mechanism: Simultaneously, a second lumen within the needle acts as a suction device, irrigating the area with saline and aspirating the debrided tissue fragments. This “vacuum and emulsification” process efficiently removes the source of the pain. A foot pedal controls the entire procedure, giving the practitioner precise control over the debridement process.
  • TenJet: This technology offers an alternative method for tissue debridement.
    • The Science: TenJet uses a high-velocity, pressurized stream of saline that acts as a “water scalpel.” This fine, powerful jet of water is directed at the diseased tendon tissue.
    • The Mechanism: The water stream’s force cuts and removes the degenerated tissue. Like Tenex, the TenJet instrument also incorporates a suction mechanism to remove debris from the treatment site.

At our clinic, we utilize the Tenex device. My clinical observations have shown it to be exceptionally effective and precise for treating various chronic tendinopathies. The procedure is typically quick, with the debridement, or “cutting,” time lasting anywhere from 30 seconds to two minutes, depending on the size of the lesion.

The Percutaneous Tenotomy Procedure: A Step-by-Step View

To give you a clearer picture, let me walk you through what a patient can expect when undergoing a percutaneous tenotomy for a condition like tennis elbow (extensor carpi radialis brevis tendinopathy).

  1. Preparation and Imaging: The patient is positioned comfortably, and the treatment area (e.g., the elbow) is cleaned and prepped using a sterile technique. We then use a high-resolution ultrasound machine to identify the exact area of tendinosis. On the ultrasound image, healthy tendon tissue appears as a bright, organized, fibrillar pattern. Diseased tissue, in contrast, appears dark, thickened, and disorganized, sometimes with calcifications.
  2. Local Anesthesia: To ensure the procedure is painless, a local anesthetic is injected into the skin and the area surrounding the damaged tendon. The patient remains awake and comfortable throughout the process.
  3. Ultrasound-Guided Debridement: The ultrasound probe is covered with a sterile sheath. I then make a minimal incision and insert the Tenex needle, guiding it in real-time directly to the degenerated tissue. The ultrasound allows me to see the needle tip as it moves back and forth within the pathologic tissue, similar to the motion used in a fenestration procedure but with the added benefit of tissue removal.
  4. Activation and Removal: Once the needle is in place, I activate the device with the foot pedal. Patients may hear a soft humming sound as the ultrasonic energy emulsifies the scar tissue. I carefully maneuver the needle tip throughout the diseased area until all the targeted tissue has been removed.
  5. Completion and Recovery: After the debridement is complete, the needle is withdrawn, and a small adhesive bandage is placed over the tiny incision. No stitches are needed. The patient can go home shortly after. Post-procedure care typically involves a brief rest period followed by a structured rehabilitation program to encourage the growth of new, healthy tendon tissue.

The Integrative Chiropractic Approach: Healing Beyond the Procedure

While percutaneous tenotomy is a powerful tool for removing the source of pain, it is only one piece of the healing puzzle. True, lasting recovery requires a comprehensive approach that addresses the underlying biomechanical and physiological factors that led to the tendon injury in the first place. As a functional medicine practitioner, I assess each patient’s unique biochemical needs. We may recommend targeted nutritional protocols and supplements to support tissue regeneration, such as:
    • Vitamin C and Proline: Essential cofactors for collagen synthesis.
    • Zinc and Copper: Minerals crucial for tissue repair.
    • Anti-inflammatory Diet: Reducing systemic inflammation can create a more favorable healing environment.
  • Targeted Rehabilitation: Post-procedure rehabilitation is critical. It begins with gentle range-of-motion exercises and progresses to eccentric loading exercises. Eccentric exercises, where the muscle lengthens under tension, have been shown to stimulate collagen production and remodeling in healing tendons (Alfredson et al., 1998).
