A New Frontier in Tendon Pain Management Innovations
Chronic tendon pain, or tendinopathy, can be debilitating and significantly affect quality of life. Traditional treatments have often provided limited or temporary relief. However, a revolutionary, minimally invasive procedure known as percutaneous tenotomy is changing the landscape of musculoskeletal care. This educational post explores the science and application of percutaneous tenotomy. It is an advanced, ultrasound-guided technique for treating chronic tendinopathies such as tennis elbow, plantar fasciitis, and Achilles tendinopathy. I will discuss the latest findings from leading researchers. Furthermore, I will explain the two primary devices used—Tenex and TenJet—which debride and remove damaged tendon tissue. I will also explain how we at Injury Medical Clinic integrate this cutting-edge procedure into our comprehensive, multidisciplinary approach. This combines chiropractic care, functional medicine, and medical oversight to create a holistic, effective path to recovery.
Hello, I’m Dr. Alex Jimenez. With my background as a Doctor of Chiropractic, Advanced Practice Registered Nurse, and certified functional medicine practitioner, I’ve dedicated my career to exploring and integrating the most effective, evidence-based treatments to help my patients achieve true health and wellness. My passion is understanding the body’s intricate connections and finding innovative solutions for complex musculoskeletal and systemic health issues.
At Injury Medical Clinic, we pride ourselves on a collaborative, multidisciplinary approach. I work alongside a phenomenal team, including our Medical Director and Collaborative Physician, Dr. Maria Guadalupe Cardenas, MD. Dr. Cardenas is board-certified in Internal Medicine and brings over four decades of invaluable experience to our practice. Her role is crucial in providing medical oversight. She ensures that our integrative protocols are safe, effective, and grounded in the highest standards of medical care. This partnership between chiropractic, internal medicine, functional medicine, and rehabilitation allows us to offer a uniquely comprehensive treatment experience. We treat everything from personal injuries to chronic conditions.
Today, I am excited to share insights into a groundbreaking procedure. It has become an essential tool in our practice for managing chronic tendon pain: percutaneous tenotomy. This post will take you on a journey to understand what this procedure is, how it works, and why it represents a significant leap forward in treating conditions that were once notoriously difficult to resolve.
For years, patients suffering from chronic tendinopathies like tennis elbow (lateral epicondylitis), plantar fasciitis, or Achilles tendinopathy have navigated a frustrating cycle of pain, inflammation, and dysfunction. The underlying issue in these conditions is not just inflammation, but tendinosis—a degenerative state in which the tendon’s collagen fibers become disorganized, weakened, and filled with scar tissue. This damaged tissue is a primary source of chronic pain. It fails to heal on its own.
Traditionally, treatments ranged from rest and physical therapy to corticosteroid injections and, ultimately, open surgery. While these treatments can be effective for some, they often do not provide lasting relief. For example, corticosteroid injections can provide temporary relief but may weaken the tendon over time. This potentially leads to further damage (Coombes et al., 2010). Open surgery, while effective, involves significant downtime, risks of infection, and scarring.
Percutaneous tenotomy is where it comes in. It is a minimally invasive, FDA-approved procedure specifically designed to target and remove the source of the pain—the degenerated tendon tissue—without the need for open surgery. This technique is now accessible to trained primary care providers like me. It allows us to offer this advanced treatment directly in an office setting.
Percutaneous tenotomy is performed under real-time ultrasound guidance, which is a cornerstone of its safety and precision. Ultrasound lets the practitioner visualize the anatomy in detail, pinpoint the exact location of the diseased tissue, and guide a specialized instrument to that area while avoiding healthy surrounding structures like nerves and blood vessels.
There are currently two leading technologies on the market for performing this procedure:
At our clinic, we utilize the Tenex device. My clinical observations have shown it to be exceptionally effective and precise for treating various chronic tendinopathies. The procedure is typically quick, with the debridement, or “cutting,” time lasting anywhere from 30 seconds to two minutes, depending on the size of the lesion.
To give you a clearer picture, let me walk you through what a patient can expect when undergoing a percutaneous tenotomy for a condition like tennis elbow (extensor carpi radialis brevis tendinopathy).
While percutaneous tenotomy is a powerful tool for removing the source of pain, it is only one piece of the healing puzzle. True, lasting recovery requires a comprehensive approach that addresses the underlying biomechanical and physiological factors that led to the tendon injury in the first place. This is where our integrative model at Injury Medical Clinic truly shines.
Following a percutaneous tenotomy procedure, our focus shifts to creating an optimal environment for healing and rebuilding. This is where my expertise as a chiropractor becomes pivotal.
Percutaneous tenotomy is more than a new technique. It represents a paradigm shift in how we manage chronic tendinopathy. By precisely removing the diseased tissue that perpetuates the pain cycle, we are not just masking symptoms. Instead, we are creating a biological opportunity for the body to heal itself. When combined with an integrative framework that includes chiropractic care, functional medicine, and structured rehabilitation, the results can be truly transformative.
My clinical observations at Injury Medical Clinic have reinforced my belief in this approach. I have seen patients who have suffered for years with debilitating pain from plantar fasciitis regain the ability to walk without a limp. Additionally, I have seen athletes with chronic tennis elbow return to their sport with renewed strength and confidence.
If you have been struggling with chronic tendon pain and feel like you have exhausted your options, I encourage you to explore whether you might be a candidate for this advanced, integrative treatment. It shows how modern, evidence-based research, when thoughtfully integrated with a holistic understanding of the human body, can provide powerful solutions and restore hope.
