Calcific tendinopathy is a condition in which calcium phosphate crystals accumulate within a tendon, causing inflammation, pressure, and significant pain. Calcific tendinopathy can occur in various body parts, such as the patellar tendon in the knee or the gluteal tendons in the hip, but it is most frequently diagnosed in the rotator cuff of the shoulder, specifically the supraspinatus tendon.
The exact cause is still debated, but leading theories point to a process of cell-mediated calcification. This isn’t a simple wear-and-tear issue. Instead, it involves a series of biological phases:

Barbotage is a minimally invasive procedure designed to break down and aspirate calcium deposits from a tendon under direct ultrasound guidance. The name itself, derived from a French term meaning “to splash” or “to wash,” describes the technique: repeated injection and aspiration mechanically wash out the calcific material.

Proper patient positioning is key. Here, the patient’s arm is placed behind their back to expose the supraspinatus tendon.
The first priority is patient comfort and safety.
Now the core of the procedure begins. A larger needle is introduced through the same anesthetized track.
This part of the procedure is methodical and can take several minutes as the practitioner meticulously breaks up the calcium.
After the barbotage is complete, practitioners commonly perform one final step.
On the ultrasound, you can see the bursa distend as the fluid fills the space, bathing the inflamed area in anti-inflammatory medication.
The barbotage procedure is a powerful intervention, but it is not the end of the treatment journey.
